Injury Reporting Requirement

In case of an emergency, call 911.

All injuries, regardless of how slight, must be reported to a supervisor immediately, but no longer than 24 hours after the injury.

These guidelines apply to all Purdue employees: Full-time, Part-time, Temporary and Student Employees (in employee status).
Students who are not in employee status follow guidelines in the following links:

  - Regional Occupational Care Center (ROCC).

Notification and Response

FIRST REPORT OF INJURY & SUPERVISOR'S INCIDENT INVESTIGATION

The First Report of Injury (FROI) & Supervisor's Incident Investigation (SII) form is available through DocuSign. The FROI form must be completed when a work-related injury occurs.

When injuries do occur, Purdue University follows the Worker's Compensation and Occupational Disease Act of Indiana which provides protection for university employees who are injured while carrying out assigned work related duties.
