Appointments
If you are seeing a provider on your own for the first time, you may wonder how to prepare, what to bring with you, and what questions you should ask. At PUSH we want to make things simple for you every single time. Please read the information below to help you prepare for your visit.
Scheduling is easy
- Schedule an appointment on the Patient Portal.
- Call Us (Request an appointment by phone) 765-494-1700, option 0.
- There are a few walk-in appointments available daily but they fill up quickly.
What to bring to your appointment:
- PUID
- Insurance Cards
- A list of current medications including prescriptions and over the counter medications
- A list of questions you have for your provider
Check in for your appointment
We ask you to arrive at the Welcome Desk located on the main floor of PUSH to check-in 15 minutes prior to your appointment time. Sometimes there is a line and this ensures that you do not feel rushed and allows plenty of time for the check-in process.
Please Note:
Students are granted a five-minute grace period for every scheduled appointment. It is extremely important that you arrive at least 15 minutes before your appointment to avoid being marked as a no-show.
- If you are late for your appointment, you will be marked as a no-show and a $25 NO SHOW fee will be added to your Bursar's account. For questions about this policy, please visit our No Show Policy page.
Cancellations
- You may cancel your appointment by calling 765-494-1700, option 0, or through the Patient Portal.
