| Protocol Type: | Enter appropriate protocol type from dropdown |
| Project Type: | Enter appropriate project type from dropdown |
| Title: | Protject Title |
| Application Date: | Field will default to the date the record was created. |
| Reference Num 1: | Used for historic protocol number reference |
