  • Are you struggling with an eating disorder?
  • Have you recently been diagnosed with diabetes, high cholesterol, or IBS?
  • Do you have a food allergy or are vegetarian and feel like you don't know what foods to choose?

If you answered yes, our Registered Dietitian is here to help you achieve your food and nutrition goals!

  • To schedule an appointment please call the Wellness Suite at 765.496.1788
  • Complete the Nutrition Assessment Form prior to your appointment.  Once you complete the form, it will be submitted directly to the Registered Dietitian.

Is a Diet Analysis for You?

  • How many grams of macronutrients (carbohydrates, protein, fat), and micronutrients (vitamins, minerals) am I taking in?
  • What is my calorie intake?
  • Am I doing a good job when it comes to my eating choices?
  • How many servings of the food groups do I need to be eating each day?

If you answered yes, our Diet Analysis program is for you!

  • To sign up, please call 765.496.1788. Once we have your information, you will be contacted by a staff member to set up a meeting.
  • Following the completion of a 3 Day Food Record, general diet recommendations will be made based on the USDA’s Choose My Plate guidelines.
  • Please note that this is not a substitute for nutrition counseling, and this may be a recommendation, if necessary.
We are located in the
CoRec Wellness Suite.
