First Report of Injury (FROI) or Near Miss
If you experience an incident or near miss, please notify your supervisor within 24 hours.
Timely reporting ensures that any needed medical care or workers compensation is not delayed. More importantly, it allows Environmental Health and Safety (EHS) to address hazards quickly and help prevent future injuries.
How to Report
- Complete the FROI or Near Miss Form
- Visit the EHS website to submit the First Report of Injury (FROI) or Near Miss form.
- Conduct an Investigation
- Supervisors should complete the Investigation Form with the employee (if possible) to determine the root cause and identify corrective actions.
Understanding the Terms
- Unsafe Condition: Broken or damaged equipment.
- Unsafe Act: Not following established safety procedures.
- Near Miss: An incident occurs, but no one is injured (e.g., an item falls near someone).
- Injury: An incident that results in harm (e.g., strain, cut or laceration).
