The IACUC Protocol Forms, also known as “Questionnaires”, are dynamic lists of questions that are the primary replacement of the traditional hardcopy application form.  Each answer an Investigator enters on a questionnaire will automatically navigate the User to the next required question.  By this method, only those questions applicable to the specific submission will be presented to the Investigator and all unnecessary questions will be excluded.  This will reduce Investigator burden and simplify the completion of protocol submissions.

Questionnaires are required for completion when available and will automatically display when appropriate.  There is no need for an Investigator to spend any time searching for or determining the appropriate questionnaire to include with their submission.

Answers to Questionnaire questions may be yes/no, value list, searches, or short answer (max. 2000 characters).

noteNOTE:  You may Save a Questionnaire at any point, exit or navigate to a different section, then retun later to complete.

 

1.  Click “Protocol Addendum” under the 2. Questionnaires heading

2.  Answer each question that is presented

3.  Click “Save and Proceed” when all questions on a screen have been answered

4.  The system will inform you when the Questionnaire is complete
