Insurance and Payments
Please upload a front and back copy of your insurance card to the Patient Portal prior to your appointment and enter your complete insurance information. At check-in a staff member will verify your name, address, phone number and insurance information. It is very important to keep this information current. We will attempt to verify your insurance coverage prior to your arrival. We are in network with most Anthem and United Healthcare insurance plans, but we will bill any insurance except Medicaid. You may want to call your health insurance company to see if services at PUSH are covered and to verify your plan’s requirements. If you have a balance after insurance, we will send it to the Bursar’s office, and you will be able to pay it through your Banner account.
- Student’s and spouses covered by the student insurance plan will pay a $15.00 copay for any services that incur a charge. Any eligible patient who is not a student will be required to pay at the time of service.
- Full-time students are eligible for free office visits for illness or injury.
- Any balances can be paid at the PUSH main floor Welcome Desk or through your Banner account.
For Patients Without Insurance:
It’s ok if you do not have insurance. Remember that full-time students can see a provider for an illness or injury for free. If you need additional services that will incur a charge, we will send the charges to the Bursar’s office, and you can pay those later through your Banner account.
International students attending Purdue University West Lafayette campus are REQUIRED to purchase Purdue's student Health insurance as a condition of enrollment, or apply for a waiver.

We currently accept check, credit/debit card, Health Savings cards, cashiers’ checks, and money orders.
The clinic will bill you directly. After 30 days, the outstanding balance will be forwarded to Purdue Accounts Receivable. Once you get a bill from them, you will have to pay them directly.
