State Immunization Requirements for Enrolled Students
Immunization Records Reminder
Students are reminded to submit proof of immunizations against rubeola (10-day measles), rubella (German measles), mumps, meningitis (strains A,C,W,Y, and B), diphtheria, and tetanus to the PUSH immunization portal. If you do not submit proof of immunization, a hold will be placed on your account and you will be unable to register for future classes.
Who:
All students taking classes on West Lafayette or Indianapolis campus must complete immunization requirements. Students who are enrolled in the Distance Learning programs will be exempt from these requirements.
What:
- Immunization records must include the student’s full name, be in English, and legible.
- The official immunization record OR a completed immunization history form can be submitted as immunization proof.
*Starting November 1, 2025, all records must be electronic/typed. We will not accept handwritten records unless they are documented on the immunization history form.

Tuberculosis Clearance
International students must have a QFT (Quantiferon blood test) or T-Spot (not PPD) with a negative result. This test must be completed in the United States and no more than three months before classes start. The test must be completed on the same date or at least 28 days after any live vaccine. Those with an abnormal TB test result will have additional steps to complete. Students not in compliance will not be able to register for future courses until the requirement has been met.
State-required immunizations are available at PUSH and through other healthcare providers.
When:
- Students starting in summer or fall must submit by June 30th.
- Those starting in spring must submit by November 30th.
*If you are unable to complete requirements prior to arriving to campus, they may be completed at PUSH after you arrive on campus. You must schedule an appointment before or as soon as you arrive to ensure you have time to finish before holds are placed. All already completed vaccine information must be submitted prior to the due dates listed above.

Frequently Asked Questions
MENINGOCOCCAL VACCINES
Both Trumenba and Bexsero require you to wait 6 months between doses. Bexsero changed their dosing schedule in August, 2024. Those who finished two doses of Bexsero prior to this date may have a dosing schedule of 28 days between doses.
TB Test
IMMUNIZATION PROOF
Students whose Purdue email address/alias changed after being accepted to Purdue will need to contact shc@purdue.edu to have their accounts manually corrected to reflect the new information. The portal is connected to the student’s email account.
Students who took previous classes through Purdue, either on campus or as an online student, will be considered "former students" until classes start. Once classes start, the student's population will correct itself and they will be able to enter immunization dates. If you'd like to submit immunization dates before classes start, please email shc@purdue.edu.
EXEMPTIONS
In the event of an outbreak of any of the vaccine-preventable diseases covered by this law on or near campus, students holding exemptions will be excluded from all campus activities, for their protection, until the outbreak is declared to be over.
Students, or their parent/guardian if the student is a minor, will write a statement indicating the reason that they cannot obtain immunizations. The statement must be hand-signed by the student and guardian, if student is a minor, and dated. This document will be uploaded as immunization proof in the Medical Clearances section of the patient portal. Once reviewed, the student will find the response in the letters section of their patient portal.
To apply for a medical exemption, a doctor or nurse practitioner must provide a statement indicating the nature and duration of the medical condition that contraindicates an immunization, as well as the specific vaccine(s) identified as detrimental to the student’s health. Medical exemptions expire when the medical condition(s) contraindicating vaccine change in a manner which permits immunization. The provider’s statement will be uploaded as immunization proof in the Medical Clearances section of the patient portal. This statement will be reviewed by our Medical Director. Once reviewed, the student will find the response in the letters section of their patient portal.
