PUSH Allergy and Immunization Services focuses on preventive health care including travel health counseling, immunizations, tuberculosis screening (TB blood test), and allergy injections. 

Travel health counseling services are available to the greater Purdue community including current students, faculty, staff, and their spouses and children (over the age of 17). For specific questions, please call 765-494-1700, option 0 during regular business hours.

PUSH provides immunizations, including the rabies vaccine to Purdue staff with departmental immunization requirements. You must have a completed Form 800 at the time of check in if your department is paying for our services. 

Allergy injections are available to eligible patients who are established with an allergist and are currently receiving immunotherapy. To initiate services, please download the forms below, and take them to your allergist. We will only accept serums shipped to us directly from the office of your allergist. All forms must be completed and returned to us prior to your first appointment. Allergy injections are administered by appointment only.
