Allergy injections are available to eligible patients who are established with an allergist and are currently receiving immunotherapy. To initiate services, please download the forms below, and take them to your allergist. We will only accept serums shipped to us directly from the office of your allergist. All forms must be completed and returned to us prior to your first appointment. Allergy injections are administered by appointment only. PUSH will not initiate immunotherapy, nor will PUSH administer rapid desensitization or “cluster shots”.
