By submitting this video, I agree to the conditions outlined below. I hereby grant Purdue University, and other organizations associated with the Purdue University Superheroes of Science and their employees, agents, successors, licensees, and assignees the absolute right and permission to copyright and/or publish images captured through video, photo, of which I may be included in whole or part, for news release, art, advertising, trade, promotion, research, or any other lawful purpose whatsoever.

I hereby waive any right that I may have to inspect and/or approve the finished product or the advertising copy that may be used in connection therewith, or the use to which it may be applied.  I understand that there is no guarantee of a release date and Purdue University reserves the right to not publish the video if it is determined to be misaligned with the objectives of the program.

I hereby release, discharge, and agree to save Purdue University, and other organizations associated with the Purdue University Superheroes of Science and their employees, agents, successors, licensees, and assignees from any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composition form, whether intentional or otherwise, that may occur or be produced in the making of said images, or in any processing tending towards the completion of the finished product. 

I hereby release, discharge, and agree to save Purdue University and other organizations associated with the Purdue University Superheroes of Science and their employees, agents, successors, licensees, and assignees harmless against any liability, loss, or damage resulting from the use of my likeness, and I hereby release and discharge any claims whatsoever in connection with such use of my likeness in the above projects. 

I am confirming this release freely and voluntarily, and I am not relying on any inducements, promises, or representations made by Purdue University or other organizations associated with the Purdue University Superheroes of Science or their employees, agents, successors, licensees, and assignees.
