COVID-19 At-Home Test Reimbursement
As of January 15, 2022, if you’re covered by a Purdue medical plan, you can get reimbursed by Anthem for the costs of over-the-counter diagnostic COVID-19 tests until the end of the Coronavirus Public Health Emergency. Diagnostic tests are performed for those who are either experiencing COVID-19 symptoms or have been recently exposed. See the Quick Reference Guide for information.
Machine Readable Files
The Consolidated Appropriations Act and Transparency in Coverage regulations require health insurers and group health plans to create machine readable files that contain the negotiated rates for in-network providers and allowed amounts derived from historical claims for out-of-network providers and make those files publicly available. A machine readable file is a digital representation of data or information in a file that can be imported or read by a computer system for further processing without human intervention.

When using the search option, the page will display links to several files, including:
- Applicable In-Network Negotiated Rate Files
- Out-of-Network Allowed Amount Files
- Out-of-Area Rates (applicable when members use providers in non-Anthem states.)
