The Office of the Registrar makes available various forms for general University use. If individual forms require specific instructions, you will be directed to an information page before your download will begin.

- Form 390-Report of Directed Credit (.pdf) (This form may be used for requests to award credit to 10 students or fewer. If your department is preparing credit awards for more than 10 students, please contact registrar@purdue.edu to learn how to prepare a file for sharing and the required cover letter.)
- Form 101A-Authorization for Individuals to Self Service Banner (SSB) (This form has been retired. Individuals with permission to request SSB access can now do so through role request by using the Form 101A link.)
- Form 101D-Authorization for Individuals to Grant Exceptions in myPurduePlan (This form has been retired. Individuals with permission to request Exceptions access in MPP can now do so through role request by using the Form 101D link.)
