Vision Coverage

Get the best in eye care and eyewear with PURDUE UNIVERSITY GRADUATE STAFF and VSP® Vision Care.

Enrollment

Those employed in a graduate staff position(s) which carries a minimum of 0.50 FTE/half time/20 hours per week or more or a graduate student with a fellowship administered as an assistantship on the West Lafayette campus are eligible for the graduate staff vision program.

Vision Coverage

Coverage Dates
For new hires and newly benefits-eligible graduate staff, your coverage will be in place following your enrollment starting with the beginning of the month in which you were hired/became eligible and terminating with the end of the academic year or the end of month in which you are eligible for graduate staff benefits.

For returning graduate staff enrolling during the annual open enrollment period, your coverage will be in place from August 1 until the end of the academic year or the end of month in which you are eligible for graduate staff benefits

  • Services related to diabetic eye disease, glaucoma and age-related macular degeneration (AMD). Retinal screening for eligible members with diabetes. Limitations and coordination with medical coverage may apply. Ask your VSP doctor for details

Glasses and Sunglasses

  • Complete pair of prescription glasses and sunglasses, including lens enhancements
  • Must purchase from VSP provider within 12 months of your last WellVision Exam

Vision Plan Cost

The cost for vision coverage is included in the graduate staff medical plan premium.

Enrollment

Graduate staff are automatically enrolled with their dependents in the graduate staff vision plan through VSP when they enroll in the graduate staff medical plan at the Academic HealthPlans (AHP) Portal. Dependents include your legal spouse and/or dependent children (until the end of the month in which the children reach their 26th birthday).

Vision ID Number

Your VSP ID number is your ten-digit Purdue University ID number. Do not add the trailing eleventh number; this is a card version number.

Access Your Benefits

Once your plan is effective, register at vsp.com to find an eye care provider and review your benefits and claims. At your appointment, tell them you have VSP. There’s no ID card necessary.
