- Call 765-494-0111.
- Go to the Center for Healthy Living portal. Login using your email and password of your choice; or new users can create an account to get started.
Are you a new patient? We encourage you to complete your New Patient Health History Form questionnaire beforehand.
No call / No Show Policy
The Center for Healthy Living makes every effort to provide prompt medical care to all of our patients. If you are unable to keep a scheduled appointment, kindly notify us 24-hours in advance.
As a courtesy to our patients and providers, failure to call or show for three appointments will result in the patient being discharged from the Center for Healthy Living. After three no call /no show instances, the patient will be asked to seek medical care elsewhere.
In addition, patients who arrive late to scheduled appointments may be asked to reschedule for another day and time if the delay will impact other patients’ scheduled appointments.
