AllOne Health Depression Screening
Via AllOne Health, the Employee Assistance Program (EAP) provider for Purdue’s West Lafayette campus, employees have access to a variety of assessments, including a depression screening survey. The survey is based on the PHQ-9 score, which is designed to provide a quick assessment of whether an individual might have signs and symptoms related to depression.
Additionally, the AllOne Health depression screening survey can be used to fulfill the Depression Screening challenge to earn incentives as part of the Your Path Wellness Program.*
Take the survey
To begin the depression screening survey, visit AllOne Health at https://allonehealthdo.mylifeexpert.com/ and log in.
Once signed in, scroll down to the center of the page and click on the “Assessments – Tools for self-evaluation” tile. Click the “Depression” tile and answer the nine questions. When finished, click “Submit” and then review the “Result Message” at the top of the page.
To locate the results at any time, click on “Profile” on the lefthand side, scroll down to “Your Courses,” “Completed Assessments” and click on the “Review Questions/Results” button (notepad icon – next to trash can icon).
Earn incentives
*To earn Your Path Wellness Program incentive funds from the AllOne Health Depression Screening, you must record the screening completion in the Your Path Wellness Portal, under “Activities,” “Depression Screening.” This must be completed in the Your Path Portal to earn incentives. If you do not certify completion in the Portal, you will not earn the Depression Screening incentive funds.
