Disclose a Disability

As part of our affirmative action efforts to employ people with disabilities, we are required by federal regulations to measure our progress toward the federally set goal of having at least 7% of our workforce be individuals with disabilities.  We report our progress annually in the Affirmative Action Plan for Veterans and Individuals with Disabilities.

Applicants for employment and new employees are asked to voluntarily disclose their disability status in SuccessFactors.  Employees may disclose their disability status or update their disability status by completing a Voluntary Self-Identification of Disability form here.

The linked DocuSign form will ask you for your full name, email address, PUID number, and current date. You will be asked to select from the following options:

  • Yes, I have a Disability or a history/record of having a Disability
  • No, I have a Disability or a history/record of having a Disability
  • I don't wish to answer

The form above is compatible with most visual accessibility tools. A hard copy or fully accessible copy of the Voluntary Self-Identification of Disability, Form CC-305 is available on request
