Carrum Health is the preferred provider of total/partial hip and knee replacement procedures for benefits-eligible Purdue employees and dependents age 18 and over covered on a Purdue medical plan.
Purdue University currently utilizes Carrum Health for other planned procedures, such as bariatric, spinal, oncology and other orthopedic procedures. Through the Carrum Health Benefit, participants have access to specialized providers and facilities selected for their expertise in certain high-risk or high-cost procedures. Expanding the program to include total hip and knee joint replacements allows us to streamline the process and make it more efficient for members to utilize.

Yes. The Total/Partial Hip and Knee Joint Replacement Program through Carrum Health is a required program for all participants covered on a Purdue medical plan. Participants who live in Indiana are required to use OrthoIndy if they live within 150 miles of one of the four OrthoIndy locations, outlined below. Participants who live outside of Indiana are required to use a participating Carrum Health center of excellence within 150 miles of their residence. Employees who wish to use a non-participating provider may request an exception from Carrum Health.
Participants will need to request an exception from Carrum Health to use any non-participating providers.
All surgery locations through Carrum will be in the HealthSync network and will count towards the tier 1 deductible and out of pocket maximum.
All employees and dependents, covered on a Purdue health plan. This includes individuals on long term disability, retirees, spouses, and individuals on COBRA.
Carrum’s Patient Care Team books and pays for travel on behalf of the patient. This includes roundtrip transportation for an in-person consult, if required. Roundtrip transportation and hotel stay for surgery trip for patient and one adult companion. Patients and companions are provided a daily stipend for remaining expenses. Employees will receive a $500 incentive for all Carrum procedures.
Employees will be issued a check, PayPal, or prepaid Mastercard to cover remaining expenses. Carrum distributes and processes 1099 forms for tax purpose to any patient who travels for medical care through the program.
- Individuals who reside within Indiana and live over 150 miles away from all four approved OrthoIndy locations, may request an exception.
- Individuals who reside outside of Indiana and live more than 150 miles from the closest Carrum COE, may request an exception.
- Individuals with a financial hardship and live over 100 miles from the nearest Carrum COE
- Individuals that cannot secure an adult companion and live over 100 miles from the nearest Carrum COE.
- Individuals with clinical reasons to use a non-participating provider. Individuals can contact Carrum directly for a detailed list of exceptions criteria.

Individuals who do not live in the state of Indiana are required to use a Carrum COE facility if it is within 150 miles of their residence. Carrum has many qualified providers throughout the United States.
Anyone with their residence in Indiana is required to use the OrthoIndy COE.
This program is a required benefit for all total hip or knee joint replacements as well as any partial hip or knee joint procedures. The Carrum benefit is also available for other orthopedic procedures on a voluntary basis. If you choose to utilize Carrum’s program for qualifying orthopedic procedures, you may be eligible for the incentive benefit, plus coverage for travel expenses. Yes, if your deductible has not already been met, you will need to work with Carrum to pay your remaining deductible amount upfront or work through Carrum on a payment plan before surgery.
Carrum offers payment plan options through Affirm Financial. Options include fixed payments, $0 late fees and options with no interest.
Plan participants can contact Carrum Health at 888-855-7806, Monday-Friday 9 a.m. to 8 p.m. ET, online at carrum.me/purdue or by downloading the ‘Carrum Health’ app on iPhone and Android devices to search for and compare participating hospitals and physicians. After contacting Carrum Health, a participant is assigned a Care Specialist to determine if the participant is eligible for the Carrum Health Benefit and provide non-medical coordination throughout the entire episode of care.
