How to complete the Intake Form

1. Look up the program you want to pursue at http://www.purdueglobal.edu
2. Click on Intake Form
3. Download a copy of the form, if you do not download the form some of the fields may not be available
   • Internet Explorer click on the save icon  to download the form
   • Chrome click on the download icon  to download the form
4. Complete all fields on the form so that your form can be processed correctly
5. Click on the email icon  within acrobat to email the form:
   • To: edbenefits@support.purdueglobal.edu
   • Insert the subject line:
     • Program of Interest - New or Returning, example: "BS in Business - New"
