Reporting Concerns

The Report a Physical or Digital Accessibility Concern Form is available for students, faculty, staff, and visitors to notify the University of any physical or digital access concerns they identify.

Physical Facilities oversees all activities associated with planning, financing, gaining approval for construction of new facilities and the Repair and Rehabilitation Program including ADA accessibility. A current list of construction notices along with the campus construction map(s) can be found on the Construction Notices and Maps webpage.

The locations of accessible entrances to campus buildings can be found of the Campus Map. Go to the Layers List tab in the upper right portion of the map, and check the box “Accessible Entry.” The blue diamonds on each building show the location of all accessible entrances.

Physical Facilities are also responsible for the following areas around campus.

To report a broken elevator, contact the building deputy for the particular building the elevator is located. A complete list of campus building deputies can be found on the Building Deputy webpage.

If a person is stuck in an elevator, however, contact the Work Request Center at (765) 494-9999 between 7:30 a.m. and 4 p.m. and the Purdue Public Safety Dispatch Center (their non-emergency phone number) at (765) 494-8221 during non-working hours.

To report a non-functioning accessible button, scan the QR code near the door to open a Fix-It Request. The Fix-it Request webpage can also be found here. Follow these steps to submit your ticket:

  • Type in the first 2 letters of the building name and then select the building you wish to report.
  • Select the floor and room. Please narrow down the location of the malfunctioning door the best you can.
  • Select “Routine repair or services” as the Repair Type.
  • Select “Doors/Keys/Locks” as the Service Type.
  • Select “Interior ADA Door-Concern/Repair,” or, “Exterior ADA Door-Concern/Repair” as the Service.
  • Provide a detailed description of the issue.

Please note that accessible doors will work after-hours if you have swipe card access to the building.
