Report all elevator incidents to the Elevator Shop

Any elevator maintenance issue should be promptly reported to the Operations and Maintenance Elevator Shop at 765-494-1744. For non-emergency work orders, individuals can also use the Fix It Service Request Portal.

In an emergency, such as being trapped in an elevator or dealing with an injury, individuals should immediately dial 911 or use the emergency phone inside the elevator to contact the Purdue University Police Department.

If an injury occurs, a First Report of Injury form should be submitted to Environmental Health and Safety (EHS).
