Employees can opt in now to obtain 2021 Tax Form 1095-C electronically

In compliance with the Patient Protection and Affordable Care Act, Purdue is required to furnish Form 1095-C to any employee who was eligible for benefits for one or more months of the calendar year.

This form contains information about health plan coverage offered to the employee and their dependents as well as health plan enrollment, if applicable. The form is typically provided by Jan. 31 of the following year (or later, if the IRS extends the deadline) and submitting it with a tax return is not required; however, it is meant to be kept with an individual's tax records. Forms 1095-C are sent by mail each year; however, employees can choose to access this form electronically in 2022.

Online furnishing provides access to the 1095-C Form earlier than the traditional paper process, and it greatly reduces the chance that the form will get lost, misdirected, delayed during delivery or misplaced after receipt. Electronic documents in general are environmentally sustainable and cost-friendly.

Employees can provide consent to receive their 2021 1095-C Tax Form electronically by filling out this online form by Jan. 5.
