Staffing transitions at Center for Healthy Living announced as One to One Health prepares to take over management July 1

In preparation to take over management of the Center for Healthy Living on Purdue's West Lafayette campus on July 1, One to One Health is finalizing its staffing roster. 

As shared in the recent Purdue Today article, one of the things to expect with the switch to One to One Health is the addition of new providers; some resident providers will move to new, outside roles as well. Effective date for the following staffing changes is July 1.

Farewell to a familiar face 

Carolyn Cooper, M.D., and current medical director at the Center for Healthy Living, is transitioning away from the center to pursue other career opportunities. The University thanks her for her years of service and dedication and wishes her the very best. Should Cooper remain in the community to practice medicine, Human Resources will alert the Purdue community as that information becomes available.

Isaacson to remain on staff

Stuart Isaacson, D.O., who joined the center’s team in January 2017, will remain on staff at the center and will serve as the medical director.

Isaacson is a board-certified family practice physician with more than 25 years of broad clinical experience. Serving as medical director of Concentra’s primary care/urgent care center in New Berlin, Wisconsin, from 2003 to 2011 and then working with Wheaton Franciscan Hospital in Racine, Wisconsin, Isaacson enjoys all aspects of primary care as well as occupational medicine.

He spent four years with U.S. Public Health Service caring for underserved patients in rural physician shortage areas and also worked with the military at both Fort Hood in Killeen, Texas, and Fairchild Air Force Base in Spokane, Washington.

In addition to medicine, Isaacson has a wide range of interests including writing, film, travel and history. He has a broad background in medical publishing and editing and served on the board of the American Medical Writers Association. He also studied film theory and criticism. Isaacson has two children -- a son who is also a physician and a daughter who works in interior design.

New providers to join the Center for Healthy Living team

* Jonathon Butler, M.D.

Butler joins the CHL team as a part-time community physician currently practicing for Indiana Health Group in Lafayette. Butler is a board-certified family practice physician with more than 20 years of experience including previous roles as medical director and chief medical officer. Butler completed his M.D. at the University of Tennessee College of Medicine in Memphis, Tennessee. He went on to complete his residency in family practice from the University of Tennessee Family Practice Residency at St. Francis Hospital.

Butler is a member of the American Academy of Family Practice, American Medical Association and American Society of Addiction Medicine.          

* Heather Gutwein, M.D.

Gutwein is a board-certified family physician with over 25 years of clinical experience, superb interpersonal skills and a reputation for excellence in comprehensive, integrated health care delivery.

Gutwein has a special interest in mental health and primary and secondary prevention. She has experience in psychiatry, comprehensive employee health, occupational health, medical missions and urgent care medicine and has been on over 17 medical missions, including hurricane relief trips to Nicaragua and Haiti. Her health care delivery philosophy is grounded in patient education, personal empowerment, facilitated self-care and faith.

In addition to spending time with her three adult children, family and friends, Gutwein enjoys reading, live music, trail bike riding, travel and playing violin and piano.

* Kimberly Porter, FNP

Porter is a recent Purdue graduate and is completing her master’s degree in the family nurse practitioner program. Porter has been a registered nurse for two years and has worked at the Lafayette School Corporation while pursuing her master’s degree. Porter graduated from Purdue with a Bachelor of Science degree in nursing and Bachelor of Arts degree in psychology. She earned her master's degree in nursing education from Purdue in 2017. Porter is also working part-time on her doctorate in nursing degree with an interest in pursuing team-based research related to autism spectrum disorders/mental health of parents and families who care for individuals with ASD.

She enjoys reading, going to concerts, spending time with family and friends and walking her English mastiff.

Medical records available

All employee medical records will be transitioned to One to One Health through secure data transfer. Employees who wish to receive personal copies of their medical records from the Center for Healthy Living can call the center to make the request. The authorization to use or disclose protected health information form must be filled out and taken to the Center for Healthy Living prior to release of records. 

About the center

The center is open 7 a.m. to 6:30 p.m. Monday through Thursday and 7:30 a.m. to 5 p.m. Friday. Lab hours are 7 a.m. to 4 p.m. Monday through Friday. For medical consultation after hours or on weekends, contact the Anthem NurseLine at 888-279-5449 for assistance or use LiveHealth Online for a virtual appointment.
