Center for Healthy Living announces upcoming closure dates: Vendor transition, holiday

The Center for Healthy Living will be closed June 30 and July 3 in preparation for the management transition from Premise Health to One to One Health, effective July 1. The center also will be closed on July 4 in observance of Independence Day.

Acute, urgent, lab and allergy/immunization appointments are available June 28 and June 29.

Appointments can be made past June 30 via the patient portal or by calling the center at 765-494-0111. All appointments after that time will automatically be transferred to One to One Health.

If a need for medical consultation arises during the closures, employees can contact the Anthem NurseLine at 888-279-5449 or LiveHealth Online for a virtual appointment.

The center will reopen at 7 a.m. July 5 and will resume regular hours of operation for medical appointments -- 7 a.m. to 6:30 p.m. Monday through Thursday and 7 a.m. to 5 p.m. Friday. Lab hours also will remain the same from 7 a.m. to 4 p.m.
