Chronic condition management assistance available via Purdue, Anthem, LiveHealth Online

Chronic diseases – according to the Centers for Disease Control and Prevention (CDC) – are conditions that last 1 year or more and require ongoing medical attention or limit activities of daily living or both. Chronic diseases such as heart disease, cancer and diabetes are the leading causes of death and disability in the United States. They are also the leading drivers of the nation’s $3.8 trillion in annual healthcare costs.

According to Candace Shaffer, senior director of benefits in Human Resources, the largest healthcare expense the University faces is chronic disease management.

“Our employees who live with chronic conditions need to know we have support systems they can utilize as they work to manage their conditions and live healthy lives,” Shaffer said.

Via Purdue, the following resources are available to help.

Condition Management at the Center for Healthy Living (CHL)

Chronic condition management is among the services available at the CHL on Purdue’s West Lafayette campus. Providers at the CHL can assist individuals with living chronic health conditions such as hypertension, diabetes, allergies, asthma and more. Additionally, allergy shots can be given at the CHL. Note: There are specific instructions that must be completed prior to scheduling an appointment to receive allergy injections by CHL staff. See the CHL’s “Services” web page for details. 

From the physicians and nurse practitioners to the health coaches, registered dietitian and worksite pharmacists, there is a care team on hand to help.

Anthem ConditionCare

ConditionCare is a no-cost health and wellness program provided by Anthem Blue Cross Blue Shield, Purdue’s medical plan administrator, designed to help individuals take control of their health, especially when managing conditions such as asthma, chronic obstructive pulmonary disease (COPD), coronary artery disease, type 1 or type 2 diabetes and heart failure.

The program provides:

  • 24/7, toll-free phone access to nurses who can answer health questions.
  • Support from health care professionals to help you reach your health goals.
  • Educational guides, electronic newsletters and tools to help you learn more about your condition.
