Center for Healthy Living offering $25 sports physicals

The Center for Healthy Living staff reminds employees that sports physicals are available at the center for only $25 for dependents covered on a Purdue medical plan. For added convenience, faculty and staff may use their PayFlex debit cards to pay for the sports physical from their health savings account (HSA) or health care flexible spending account (FSA).

Sports physicals are not eligible on the Purdue medical plan, so they won't count toward the employee's deductible or out-of-pocket maximum.

"As summer sports camps and events near, it is a good time for parents to think about scheduling any needed sports physicals,” says Millie Fenneman, practice administrator at the center. “Everyone getting a sports physical will need to bring the required IHSAA form to the appointment."

Call 49-40111 to make an appointment. If the sports physical will be the child's first visit to the center, Fenneman recommends filling out the patient intake form in advance and bringing it to the appointment. 

"While a sports physical does not replace an annual preventive exam for a child with his or her regular pediatrician, it’s a good idea to get both," Fenneman says.

The center is open 7 a.m. to 6:30 p.m. Monday through Thursday and 7 a.m. to 5 p.m. Friday. Lab hours are 7 a.m. to 4 p.m. Monday through Friday. For assistance after hours or on weekends, contact the Anthem nurse line at 888-279-5449 for assistance.
