Forms Required for the MRI Scan Sessions
- For all subjects, please use the Purdue MRI Safety Screening Form. A parent/gaurdian must complete and sign for minors.
- Please fill out and sign the Siemens Nominal Configuration Form prior to leaving.
In case of an incident or injury while at the MRI Facility, please complete the Report of Injury Form.
