New insurance cards for employees who made plan changes

Anthem ID cards for Purdue employees were mailed out Dec. 4, Dec. 5 and Dec. 8. Please note that ID cards were sent only to employees who made plan changes for 2015. All other employees will continue to use their existing ID cards.
As a reminder, the Center for Health Living office visit fee for 2015 has changed from $45 to $40 under the Purdue Health Plan Plus HSA 1 and HSA 2 plans. However, employees who did not make plan changes will retain their existing cards that reflect the former $45 CHL office visit fee unless they choose to request new cards.
