Welcome to CHL Telehealth

We’re writing to welcome you to a new integrated telehealth program led by the Center for Healthy Living (CHL). This program is an effort to provide convenient and timely access to acute care and healthcare consultation.

We believe that access to medical care shouldn’t be hindered by financial decisions or arbitrary barriers. To overcome those hurdles, we have staffed a dedicated care team that is now accessible to you 24/7 via text message, video chat, or phone call. You have unlimited access to our providers and care team. They can provide treatment for acute needs, write prescriptions, provide medical advice, schedule appointments, make referrals, and provide timely follow-up. Telehealth is available at no cost to you in 2024.

Meet Your Providers:

Mary Evangelista, FNP-C: I am a 2008 Purdue grad and have been caring for patients as a nurse and nurse practitioner since 2011. I live in Crown Point and have experience in both emergency medicine and family practice.  I enjoy spending time with my husband, daughters, and our two dogs (a black lab/pit rescue and a cavalier spaniel).

Amanda Vagelatos, FNP-C: I love the CHL Telehealth program because of the collaborative approach to delivering quality care, connecting with patients wherever they are in their day. When I’m not working I love spending time with my husband and our 3 children. We spend a lot of time at sport practices and games. We love to travel, especially to the beach.

What would I use the care team for?
Think of the CHL Telehealth team as your front door to healthcare. If you have a healthcare related question, need, or issue, you will be able to simply text them and receive care. The team can conduct video visits, write prescriptions, offer advice, scheduling support, benefits navigation, and referrals as necessary.

How do I use the CHL Telehealth service?
Simply text the CHL Telehealth team at (765) 705 0584. The care team will ask each new patient to complete a Registration Form which initiates HIPAA compliant consent documentation for communication via text, phone and video calls as well as providing important health history. You may also choose to download our app to communicate with the telehealth team.

When should I use it?
Mary and Amanda are available 8am to 5pm EST with back up support provided 24/7 from the One to One Health Virtual Care Team. If you contact the team after hours you will likely get a different licensed medical provider, but they will coordinate with Mary and Amanda to ensure continuity of care and appropriate follow-up.

Will using the service cost me anything?
There are no co-pays, visit charges, bills, or insurance charges of any kind for 2024. If the CHL Telehealth refers you to in-person care, you will be billed as normal for any in-person care received.

Who is eligible?
All benefit-eligible Purdue employees and dependents, age 2 and above, covered on a Purdue medical plan are eligible to use the Center for Healthy Living Telehealth Program.
