Center for Healthy Living announces new electronic health record system effective July 20

One to One Health, which operates the Center for Healthy Living (CHL) locations, is transitioning to a new, more innovative electronic health record (EHR) system — Elation Health — effective July 20. The move to the new platform helps enhance communication between care teams, continuity of care, and ongoing enhancements to digital health services and patient engagement tools.

Existing medical records will undergo a secure and complete migration from eClinicalWorks into Elation Health as part of a carefully managed system transition process. The care teams at the CHL locations will maintain complete and uninterrupted access to their patients’ health records throughout the transition.

Beginning July 20, patients will access their health information through Elation’s secure patient portal, Passport. The new patient portal and different appointment reminders will be the main changes noticeable to patients.

Action needed

As part of the ongoing modernization and patient-experience improvement efforts, patients utilizing the CHL’s Telehealth program will be able to use the TextCare Patient App beginning July 20 as well.

Patients will begin receiving communications from One to One Health this week with detailed information about the transition including what to expect, how to login and how to schedule an appointment.
