2025 IRS Form 1095-C now available online

The IRS requires Purdue, as an employer, to report whether an employee and dependents (if applicable) had health coverage in 2025. Therefore, employees receive Form 1095-C, which details whether they were offered health coverage by Purdue and during which months. The IRS does not require a copy of your 1095-C form to complete a tax return.

Due to the Paperwork Burden Reduction Act, signed into law Dec. 23, 2024, employers are no longer required to automatically mail paper copies of Form 1095-C. Paper forms will only be mailed upon request.

Forms are now available online through the Benefitfocus portal. To download a copy of the form:

  • Log in to Benefitfocus via the OneCampus portal
  • Select “Benefits” in the upper right corner
  • Under “Benefit Reports,” choose “1095 tax documents”
  • Select “Download” to save or print the form

Individuals who have questions or would like a paper copy mailed to their home should contact Human Resources at 765-494-2222 or via email at hr@purdue.edu. Please include “2025 1095-C” in the subject line of the email when requesting a paper copy.
