Human Resources announces new process for requesting FMLA, PPL for Purdue, Purdue Global, PARI employees

Human Resources is offering a new and more efficient way for Purdue employees (including graduate staff), as well as Purdue Global and Purdue Applied Research Institute (PARI) employees, to request family and medical leave (FMLA) and paid parental leave (PPL).

Effective now, employees can request FMLA or PPL by completing an online request form. This streamlined process aims to improve the speed and accuracy of leave requests while making the entire process more transparent and straightforward for the employee. The online request form can be submitted by the employee, supervisor or employee’s representative.

To access the form, please visit the Leaves/Time Off/Disability webpage and see the Leave/Time Off Requests section. The form is also available on the FMLA and PPL webpages.

When the intake form has been completed, the employee will receive a confirmation message, and the request will be sent to the HR department for review and processing. Once the leave case has been opened, the employee, supervisor, HR business partner and Business Office/Employment Center will receive confirmation and will be made aware of their case number and leave representative.  General questions regarding FMLA can be directed to Human Resources at hr@purdue.edu or 765-494-2222 or to the appropriate regional campus HR office.
