Anthem ConditionCare provides a no-cost program to help individuals manage chronic conditions

ConditionCare is a no-cost health and wellness program provided by Anthem Blue Cross Blue Shield, Purdue’s medical plan administrator, designed to help individuals take control of their health, especially when managing conditions such as asthma, chronic obstructive pulmonary disease (COPD), coronary artery disease, type 1 or type 2 diabetes and heart failure.

“The largest healthcare expense we face is chronic disease management,” said Candace Shaffer, director of benefits in Human Resources. “As a reminder, Anthem’s ConditionCare program provides our employees who live with these types of chronic conditions another support system to utilize as they work to manage their conditions and live a healthy life.”

The program provides:

  • 24/7, toll-free phone access to nurses who can answer health questions.
  • Support from health care professionals to help you reach your health goals.
  • Educational guides, electronic newsletters and tools to help you learn more about your condition.

If you are interested in utilizing the ConditionCare program, call Anthem toll free at 866-962-0963 to get started.  Calls with Anthem are always confidential; the Anthem rep will ask for the individual’s name and date of birth to ensure identity.

You may also receive a call from Anthem. As part of its normal business practice, Anthem may contact policyholders on occasion to share important health information, give appointment reminders or promote a no-cost Anthem health and wellness program, such as ConditionCare.

“Should an employee receive a call from Anthem, we want to make sure they know it is not a sales call and calls only occur as a help resource,” said Shaffer. “Any suggestions or programs Anthem recommends during a call are already included in an employee’s Purdue health plan. These calls serve as another helpful source for our employees when it comes to managing their health and wellness.”

Employees also can contact Anthem at the customer service number listed on the back of their ID card at any time to discuss the health programs and services offered.
