Medical Plans, Voluntary Benefits & More

Your physical health is critical to your total well-being, so we offer medical benefits, preventive care, treatment programs and ongoing health education to give you the foundation you need for physical health and total well-being.

I n this email, you’ll find information on:

  • Medical plan options for 2022

  • New medical ID cards

Important information:

  • Purdue benefits website is updated with 2022 benefits information, including a dedicated open enrollment website. 
  • The 2022 benefits guide is now available! Get a jump start on preparing for our open enrollment later this year.
  • Oct. 26 – Nov. 9: Open enrollment period for 2022 benefits

Medical Plan Options

Your medical plan options for 2022 remain the same. You have a choice from three Consumer Driven Health Plans, or CDHPs.

All three plans have:

  • Free preventive care with in-network providers
  • Free Affordable Care Act (ACA) generic preventive prescription medications and generic non-preventive medications no more than $10, once deductible is met
  • Lower costs with the Tier 1 (HealthSync) Network, Center for Healthy Living and Fort Wayne Campus Health Clinic providers
  • Health Savings Accounts (HSAs) where you and Purdue set aside tax-free dollars to help pay for medical expenses*

*Employer contributions made into HRAs for those ineligible for an HSA

How a CDHP Works

  • You pay lower premiums, deducted from your paycheck each pay period.
  • You can make tax-free contributions to your HSA, also via payroll deduction.
  • You visit a provider when you need medical care.
  • Your provider bills you and you pay the bill directly. (You can pay using your HSA or HRA.)
  • You continue to pay costs until you meet your deductible.
  • After you meet your deductible, you and Purdue share the cost of any medical service.
  • You stop paying for eligible medical care if you reach your out-of-pocket maximum. At that point, costs are 100% covered by Purdue.

New Medical ID Cards

All employees covered by our medical plans will receive new medical ID cards in January. The new cards will have a new group number, so it's important for you to use your new card beginning Jan. 1. If you visit a provider or need to fill a prescription before you receive your card, you can download Anthem’s Sydney app for a digital version of your ID card.

Anthem’s Sydney app allows you to manage your healthcare right from your phone.

  • Check claims: See what’s covered and what you owe.
  • See benefits: Check what your plan covers 
  • Get your ID card: Share, fax or email your ID card right from your device.
  • Find a doctor: Look for doctors in your plan.
  • Get medicine: Refill your prescriptions online.
  • Estimate costs: Compare costs and quality for common procedures.
  • Get discounts: Save on health-related products and services.

Voluntary Benefits for Physical Health

Voluntary benefits are intended to supplement your core benefits from Purdue. They can give you extra peace of mind that if an unexpected situation occurs you will have additional financial protection. Plus, they are:

  • Offered at very competitive rates

  • Work in conjunction with your medical and savings plans

Below are the highlights of the voluntary benefits we offer that support your physical health:

Accident Insurance
(Voya)

  • Pays you directly for accident-related expenses (i.e. medical treatment, hospitalization, diagnostic testing, follow-up care, transportation, lodging)
  • Used in addition to Purdue’s medical and disability benefits
  • No pre-existing condition limitations
  • Two plan level options (and family coverage) available
  • Paid through payroll deduction

Critical Illness Insurance (Voya)

  • Lump sum benefit is payable to you if you or a covered family member are diagnosed with a covered critical illness (i.e., heart attack, stroke, cancer, coronary artery bypass graft)
  • Used in addition to Purdue’s medical and disability benefits
  • Coverage levels: $10,000, $20,000 or $30,000
  • Spouse/dependent coverage: 50% of selected employee benefit
  • Wellness benefit paid annually if you complete a covered health screening
  • Paid through payroll deduction

Supplemental Hospital Insurance (Voya)

  • Pays you for hospital stays due to accident or illness
  • Flat amount for a hospital admission plus a per-day amount for subsequent days of hospitalization
  • Includes coverage for maternity hospitalization
  • Used in addition to Purdue’s medical and disability benefits
  • No pre-existing condition limitations
  • Two plan level options (and family coverage) available
  • Paid through payroll deduction

Long Term Care
(Mutual of Omaha)

  • Covers services when provider certifies you are unable to perform at least two of the six activities of daily living, or continual supervision needed due to a severe cognitive impairment
  • Covers services received in home, assisted living facility or a nursing home
  • Ability to qualify for long-term care insurance is based on your current health
