To request coverage, fill out the
RM05
request form and forward to the Risk Management Office. This form needs to be submitted the month in which the program starts.
Submit a roster of names, addresses, and parent information once the camp has started.
To submit a claim, please complete the
RM26
form for each participant that sustained an injury. Follow the detailed directions on that form for submitting a claim directly to American Income Life. The faculty member or camp counselor needs to sign the form. The claim form and any bills should be sent directly to American Income Life via Filelocker to the e-mail address provided on the claim form directions. Please make sure to include the home mailing address of the camp attendee or participant.
Arrange for the appropriate medical treatment. The medical facility should be told that the American Income Life policy is primary to any other medical policy. All medical invoices should be forwarded to the Risk Management Office.
