ACTIVITY REFLECTION PROMPT
Write up a short reflection after you've completed an activity. Submit it to the panel for grading on a pass/fail basis, and earn the point(s) associated with the activity!
PROGRAM REFLECTION PROMPT
When you've completed all your activities, submitted your reflections, and earned 7 points, use this form to write a one-page reflection on what you've gained by participating in this program. Submit it to the LDP for grading, and earn your Leadership Award!
VERIFICATION OF COMPLETION FORM
