Collection Information

If submitting specimens from multiple animals, 1) label each tube sequentially (e.g., 1, 2, 3…) to correspond to the specimen/animal order on the submission form; 2) for each specimen, list animal IDs on submission form; and, 3) if >10 animals per submission, please submit an electronic spreadsheet with animal IDs (in sequential order) – this will facilitate accessioning and data entry, reducing turn-around time. If multiple serology tests are requested on any one serum sample, minimum amount is 1-1.5 ml.

Refrigerate specimen(s) until shipped.

Shipping Information

If shipping different tissue types, clearly label each container with owner and animal ID (if applicable) and sample/tissue type. Ship specimens overnight with an ice pack. DO NOT SHIP OVER WEEKEND OR HOLIDAY. **Fee: $10.45 - carbon form submissions; $9.50 GVL/VSPS submissions (no accession fee for either).

ADDL West Lafayette location tests will be set on Tuesday and Thursday. Plates must incubate for 24-48 hours once set prior to reading and reporting results. Delay in results if confirmation at NVSL is needed.

The most current version of the VS 10-11 form is the February 2018 revision. USDA does not accept older versions. Electronic versions of the form that are approved by USDA will be accepted.
ADDL is able to process electronic EIA forms by VSPS and Global Vet Link.

What actions are required?

Category II submitting veterinarians:

Destroy any version(s) of this form printed prior to February 2018.

Current EIA VS 10-11 forms are available free of charge from the Indiana State Board of Animal Health. Order online or contact BOAH: 317-544-2400 or by email at animalhealth@boah.in.gov.

Accurately and fully complete the form according to instructions and uniquely identify the animal. Blank fields are not acceptable, use NA when appropriate.

The ADDL can only accept accurately and fully completed, official forms with sample submissions. Only forms received from Category II accredited veterinarians authorized to perform those duties in the state the sample was obtained will be accepted. Veterinarians that submit an outdated form after May 1, 2020, will be contacted and asked to re-submit the new version, which will result in a delay of testing.
