Today is the final day of open enrollment in Purdue’s new life insurance program

Today is the deadline for enrolling in Purdue's new life insurance program. To enroll, get your user name and password from the packet you received at home from Minnesota Life. Then enroll online at www.lifebenefits.com.

If you do not have your password, you will need to complete a paper enrollment form and beneficiary designation. These forms are available at the Staff Benefits Web site at http://www.purdue.edu/hr/Benefits/newlife.html. Paper enrollments need to be mailed to Minnesota Life and must be postmarked no later today (May 8). 

Whether you are enrolling online or by paper, no exceptions to the May 8 deadline will be made. 

Faculty and staff who do not enroll will default to the following coverage: 

* 1.5 times salary in employee term life insurance.

* $15,000 in employee accidental death and dismemberment (AD&D) coverage.

* No spouse/same-sex domestic partner term life or AD&D coverage.

* No child term life or AD&D coverage.
