Action required by May 13 to avoid losing medical coverage on dependents

Faculty and staff members who cover dependents on their Purdue medical plan have until May 13 to provide required dependent information and verification documents. Dependents will be dropped from medical coverage if employees do not submit the required documentation by the due date.

The needed items are outlined in the Dependent Eligibility Verification Package sent to employee homes in early April by Mercer, the human resources consulting firm Purdue is partnering with to conduct the verification program.

Though the verification package includes a return envelope for submitting the required items, faculty and staff may wish to take advantage of the secure fax option instead. Documentation sent to the secure fax number goes directly into the Mercer database without producing any paper to be retrieved on the receiving end. Faxing may also make it easier to meet the May 13 due date. 

The fax number is included in the Dependent Eligibility Verification Package. The package also provides the number to the Purdue Dependent Eligibility Verification Service Center for employees who need assistance.
