April 30: Last day for HealthPath Questionnaire
There are two days left to complete level one of Healthy Purdue 2008.
At 8 p.m. EDT on Wednesday, April 30, the online HealthPath Questionnaire (HPQ) will be shut off. To complete the questionnaire before the deadline, go to www.purdue.edu/healthypurdue and click StayWell's logo.
Those with questions may call StayWell's HelpLine at (800) 926-5455. HelpLine hours are Monday through Thursday, 9 a.m.-9 p.m. (EDT); Fridays, 9 a.m.-7 p.m.; and Saturdays, 9 a.m.-1 p.m. Voicemail is available after hours.
