Last day to request paper version of HealthPath Questionnaire
Today is the last day those participating in Healthy Purdue 2008 can request a paper HealthPath Questionnaire (HPQ) from StayWell.
The paper HPQ must be postmarked by April 30. Those who complete the paper HPQ should receive a results booklet from StayWell approximately three weeks after sending their completed questionnaire. Call StayWell's HelpLine at (800) 926-5455 to request a paper HPQ, or with questions about the results booklet.
This week, the School of Nursing has openings for wellness screenings at 8:10-11:30 a.m. and 1-3:20 p.m. Friday (April 18) in Room B-5, Johnson Hall of Nursing.
To register, go to www.purdue.edu/worklife and click "Register Now." Once logged in, look for "Wellness Screening H, April 2008, School of Nursing."
StayWell also has screening openings this week. Once logged in, look for "Wellness Screening L, April 2008, StayWell. "
StayWell screening dates are:
-- April 17, 8:30 a.m.-11:30 a.m. and 1-4:30 p.m., STEW 302-306.
-- April 18, 9 a.m.-1:30 p.m. and 2:30-5 p.m., KRAN, Drawing Room.
-- April 19, 8:30 a.m.-1 p.m., STEW 218.
