Complete a screening, HPQ for Healthy Purdue
This is the last month for those participating in Healthy Purdue 2008 to complete their HealthPath Questionnaire and earn the $150 (before taxes) incentive.
Screenings are still available on the West Lafayette campus through the School of Nursing:
-- April 11, 3-3:30 p.m., JNSN B-5.
-- April 14, 8:30-11:30 a.m. and 1-3:20 p.m., STEW 313.
-- April 15, 8:10-11:30 a.m. and 1-3 p.m., STEW 313.
-- April 16, 18, 21, and 25, 8:10-11:30 a.m. and 1-3:20 p.m., JNSN B-5.
-- April 29 and 30, 8:30-11:10 a.m. and 1-3 p.m., JNSN B-5.

Look for "Wellness Screening L, April 2008, StayWell" in WorkLife Programs' online registration system.
Those interested in a screening are encouraged to register at least 24 hours in advance of the appointment time.
The screenings are free. Participants must bring their Purdue ID.
Participants are also welcome to see their health care provider to complete a screening. Use the wellness screening form available at www.purdue.edu/healthypurdue/forms.htm.
