Healthy Purdue deadline approaching
Healthy Purdue participants must complete a wellness screening and the HealthPath Questionnaire (HPQ) by April 30 to earn $150 (before taxes) and become eligible to complete the apples for an additional $250 (before taxes) incentive.
Screenings are still available on the West Lafayette campus. For a list of available dates and times, go to www.purdue.edu/worklife and click "Register Now."
Participants will use their Purdue career account and password to log in. After logging in, click on "Wellness Screenings" to sign up for a time.
Regional campus screening events have been completed.
Participants also can see their health-care provider to get the necessary values for the HPQ. Participants can print out a wellness screening form to take to their health-care provider at www.purdue.edu/healthypurdue. The form is not needed for screenings completed on campus.
After completing a screening, participants can fill out the HPQ online at www.purdue.edu/healthypurdue. Click on the StayWell logo. The HPQ will be available until 8 p.m. Eastern Time on April 30.
A hard copy of the HPQ is available by calling StayWell at (800) 926-5455. Requests for a hard copy must be made by April 14.
