Description

Our new Personal Health Manager Kit is an easy-to-use tool designed to help patients organize and keep track of appointments, medication, test results, insurance, bills, and various other logistical and practical details associated with cancer. The kit's content can be personalized to provide information and resources which will help address the needs of a particular cancer patient or caregiver. Kits order and shipment is free of charge.

Will assist with transportation for any cancer treatment.  It requires 4 business days advance notice.  Please provide the date, time, location and duration of appoint.  They will work with you if you have a regular schedule of appointments needed.  This program is 100% based on volunteers and patient must be able to get in and out of vehicle without help.

Description

For those who cannot use the regular fixed route bus services because of a physical/ mental disability.  Must fill out an application form & have Doctor’s order that you are not able to ride the regular bus services, in order to be certified for this service.  Call for an application form.  Once certified, you must call at least one-two weeks in advance of your trip. The service is available Monday through Sunday, hours vary.

Cost:  $2.00 for one trip; round trip, $4.00. An assistant may ride free. You may purchase a ticket for $20 for 10 rides. Drivers will assist into & out of the seats & bus & will assist with mobility aids & packages less than 25 pounds.

Prompt Ambulance Central is headquartered in Lafayette, and is a division of Prompt Medical, the largest EMS Agency in the State of Indiana with Corporate HQ in Highland, IN. At Prompt Central we are a 24/7/365 medical transport agency serving the hospitals, care facilities and communities in a 14 county region around Greater Lafayette. Prompt Central puts 13 ambulances on the road every day, as well as 6 wheelchair vans. We provide acute and emergency transport, Advanced Life Support, Basic Life Support, Bariatric transport, as well as wheelchair and med-car transport.

Prompt Central is also the E-911 Provider for Cass County and Logansport, as well as for Jasper County and Rensselaer. Our parent division, Prompt Medical is the E-911 and acute medical transport provider for most of Lake County, IN.

Our medical field staff includes Paramedics, Advanced and Basic EMTs, as well as our Medical Director, Dr. Edwin Eppler, our Director of Operations, Nathaniel Metz, NREMT-P, our Patient Care Coordinator Robert Beck, LPN, and our Education Coordinator Allen Borck, NREMT-P, MA Education. Prompt provides EMS services for public events such as the County Fair and the Mosey Down Main Street, as well as for faith based and not for profit organizations hosting 5k/10K events, festivals, etc. We also provide EMS Services for racing events and any event that requires medical response.

Prompt Central has its own State Certified Prompt Institute of Paramedicine, where we provide instruction and certification for aspiring EMTs and Paramedics. Many Purdue students have worked for Prompt over the years, and we welcome those who want to pursue employment and career opportunities in the field of EMS.

In addition, each summer Prompt Central, in partnership with NCI-AHEC, hosts the annual Discover EMS Camp for high school youth interested in pursuing careers in EMS.

Description

Patient Navigation Services available for those diagnosed with Breast or Cervical Cancer once enrolled into the program. Cancer Wellness Program available as described in Nutrition and Wellness section. Funding provided for Mammograms, Pap tests and other diagnostic services/treatment related to breast and cervical cancers provided at no charge for eligible women. Referrals to volunteer wound care nurses that work with clients enrolled in our program to provide dressing changes and wound care assistance. Wound care supplies in limited quantities available to clients with breast cancer once enrolled in our program.

Program serves 23 Indiana counties. Spanish speaking staff available. “ The YWCA does not provide medical advise, consultation or direct medical care. All referrals are made for informational purposes only.”
