Description

Our new Personal Health Manager Kit is an easy-to-use tool designed to help patients organize and keep track of appointments, medication, test results, insurance, bills, and various other logistical and practical details associated with cancer. The kit's content can be personalized to provide information and resources which will help address the needs of a particular cancer patient or caregiver. Kits order and shipment is free of charge.

Will assist with transportation for any cancer treatment.  It requires 4 business days advance notice.  Please provide the date, time, location and duration of appoint.  They will work with you if you have a regular schedule of appointments needed.  This program is 100% based on volunteers and patient must be able to get in and out of vehicle without help.

Mission: To support the non-medical needs of area cancer patients in collaboration with care providers and other community resources.

Service Area:  11 North Central & West Central Indiana counties - Benton, Cass, Carroll, Clinton, Fountain, Jasper, Montgomery, Newton, Tippecanoe, Warren, White

Call CCN direct or be referred by health care provider, family or  friend  to become a Community Cancer Network client

Description

The Mobile Pantry Outreach Program directly serves clients in areas where food is needed in an effort to supplement other hunger relief agencies in that area. LUM is an organization of 47 local churches that serves as a social safety net in our community for families and children with critical needs. This Program is a program through the American Cancer Society, and the lines are open 24/7 to help with your questions.  This is a free service for women who are currently undergoing treatment for a cancer diagnosis (chemotherapy, radiation therapy or other forms of treatment), Look Good Feel Good is designed to help women gain the skills and confidence for increased independence in personal appearance. Registration is required. Local facilities offering the program are listed below. Veterans Affairs

Description

Benefits to veterans with disabilities either service-connected or non service-connected. Aid and Attendance benefit to assist with paying for home health services may be available. For veterans who receive services through the VA healthcare system, there are other home health services, etc. available, including a caregiver resource line.

YWCA Greater Lafayette Women's Cancer Program

Description

Patient Navigation Services available for those diagnosed with Breast or Cervical Cancer once enrolled into the program. Cancer Wellness Program available as described in Nutrition and Wellness section. Funding provided for Mammograms, Pap tests and other diagnostic services/treatment related to breast and cervical cancers provided at no charge for eligible women. Referrals to volunteer wound care nurses that work with clients enrolled in our program to provide dressing changes and wound care assistance. Wound care supplies in limited quantities available to clients with breast cancer once enrolled in our program.

Program serves 23 Indiana counties. Spanish speaking staff available. “ The YWCA does not provide medical advise, consultation or direct medical care. All referrals are made for informational purposes only.”

In partnership with Purdue University Department of Health and Kinesiology; a fitness program to improve the quality of life of cancer survivors is offered at various times throughout the year. For class schedule and registration contact the YWCA.
