Description:

Carroll County Senior & Family Services provides transportation services to seniors and disabled of Carroll County. Medicaid is billed for those eligible or a donation amount is due at the time of service. The rates are as follows:
IN TOWN           $5-$10
IN COUNTY       $10-$15
LAFAYETTE/KOKOMO/MONTICELLO/LOGANSPORT  $ 20-$30 per each
INDIANAPOLIS  $60-$80
(The range is based upon the length of appointment. This will be explained when trip is scheduled)
