Nursing Services
Nurses provide medical care under a physician's plan of treatment, and help patients obtain other community services as needed. Nurses are available on a "PER VISIT" bases, or hourly (from 1 to 24 hours a day)
Case Management
Assessments of a client's total care needs; care planning, assistance in locating and purchasing services/supplies and coordination and monitoring of services.
Special Services
Some Indiana Home Care Plus nurses provide highly specialized care for: cancer patients, terminally ill patients, patients with lung disease (including ventilator care), patients requiring intravenous therapy and new mothers and infants.
Personal Care
Specially trained aides help patients perform everyday tasks made difficult by illness or injury.
Rehabilitation Services
Physical, Occupational and Speech therapy help regain independence after an illness or injury, or developmental problems or aging. These therapists concentrate on re-teaching patients to perform those activities most meaningful to them.
Social Services
Social Workers help families with emotional, social and financial problems.
