How do I file a claim?


If you are seen at PUSH, claims will be submitted for you at your request. If you are seen at any other location, and a claim has not been submitted for you, the following must be included when submitting to insurance:

  • Claim form
  • Medical and/or hospital bills
  • Insured person's name
  • Address of insured
  • Patient's name
  • ID number
  • Name of the college under which the student is insured

Claims should be submitted to insurance within 90 days of the date of service. Claim forms, including all documentation, should be mailed to:

How can I get a claim form?

Claims forms are available at the Student Health Insurance office or you may print a claim form here.

What do I do if I receive a bill?

You will have access to an itemized statement after you visit PUSH (see here for further information). It will show any charge that may be your responsibility (a co-pay, or excluded charge) and any charges that have been submitted to insurance. If you receive any other bill that appears not to have been submitted to Student Insurance, you may mail the original bill to Student Insurance directly along with a claim form. Keep a copy of everything submitted for your records.

How do I find out about a pending claim?

Check your online account with United Healthcare Student Resources first. If you still have questions, you may contact Student Insurance for all claim inquiries at 1-888-224-4754.

I received information from Student Insurance, what do I do?

It is very important to complete the requested form and to supply the additional information in order for Student Insurance to continue to process your claim. If you fail to supply the requested information, your claim may be delayed. You will also receive an EOB (explanation of benefit) from Student Resources after they have completed processing of your claim.

My claim has been denied. How do I file an appeal?

You may submit a letter to the insurance company requesting a review of your claim. Include any explanation that will help them to better understand the situation/complaint. Copies of the medical record should be included if Student Resources has not previously requested. Send to:
