Insurance Information Necessary to File a Claim

It is important that you keep the health center informed of any insurance information changes. A student should carry a copy of the health insurance card at all times. Additional copies may be requested from your insurance company. Our insurance information form may be completed and faxed or mailed to the Health Center with new information. Please attach a copy of the card.

We do not bill to Medicare or Medicaid. Students should contact their PMP (Primary Medical Provider) to determine where they can be seen. The local Lafayette hospitals do participate in the Medicaid program.

If your insurance policy is with Anthem/Blue Cross/Blue Shield, we will file a claim at the patient’s request. However, a bill will be sent to the patient as this insurance company most often does not reimburse the University directly. Any amounts that are not covered by insurance are the responsibility of the patient.

If a student requests charges be filed to insurance, but there is no insurance on file, an email will be sent requesting this information. If not received in a timely manner, the charges will be sent to the Bursar for billing to the student. Filing a claim after charges are sent to the Bursar becomes the responsibility of the student.

We are OUT-OF-NETWORK for all insurance companies, except for the university sponsored Student Resource insurance plan.
