Resources for Reporting Student Issues

THESE REPORTS RELATED TO STUDENT WELL-BEING SHOULD BE UTILIZED TO REPORT ISSUES ON STUDY ABROAD PROGRAMS, REGARDLESS OF THE ACTUAL LOCATION OF THE INCIDENT.

Student of Concern Report

This report goes directly to the Office of the Dean of Students and can be utilized for any level of student behavior issue that may be cause for concern, from sudden changes in mood to threats or acts of violence.

Title IX Report

This report goes directly to the Office of Institutional Equity and the appropriate Title IX coordinators. Please note that faculty are usually considered mandatory reporters for Title IX incidents.

CSA Report (Clery Act)

This report goes directly to the Clery Compliance area of the Purdue University Police Department. Please note that Purdue University is required by federal law to report on campus crime statistics.

General Incident Report

This report goes directly to the Study Abroad Office to report any type of incident.  This is a general incident report providing space to document a variety of situations.
